I have noticed that some old threads have the 'new' icon by them and some of the newer ones do not. Could anyone explain how this system works?

Threads that have been updated since you have last visited have the new thing by them, threads that you have checked don't or have visited and ignored do not. If there are too many and they are annoying there's a button on the page that you can click labeled "mark all threads as read"

And sometimes, lots of posts mark themselves as new for no apparent reason (for me). Must be a forum bug.

Yeah, same here. It's probably a bug, a lot of the locked threads seem to do that for me.
